Florida Learning and Action Network (LAN) and Asthma Care Team (ACT) Programs
Two projects working synergistically to improve collaboration between healthcare and public health agencies to progressively expand sustainable and comprehensive asthma control service networks across Florida. One project involves statewide, sector-specific LANs that include managed care organizations and providers. The second involves community-based, multi-sector ACTs that include providers, schools, hospitals, pharmacies, local public health agency and housing entities.
Who
The LAN engaged managed care organizations and providers. The ACT engaged multi-disciplinary and multi-sector representatives including providers, schools, hospitals, pharmacies, local public health agency and housing entities.
Our Role
Provided quality improvement expertise in the planning and development of learning collaboratives to engage different stakeholders and community members in improvement work to enhance care coordination and asthma management.

Patient Safety Bundles
From the Alliance for Innovation on Maternal Health. PSBs are collections of evidence-informed best practices that address clinically specific conditions in pregnant and postpartum people. The NNPQC helps provide TA and support to PQCs in adopting the core AIM Perinatal Mental Health Bundle.
Perinatal Quality Collaboratives
The CDC’s landing page for PQCs, including helpful infographics, videos, and links to help explain what PQCs are, how they work, and stories, learnings, and publications that have come out of the state PQCs.
