Spread of Medical Home Concept
A project to improve the quality of health services received by children and youth with special healthcare needs through the widespread implementation and diffusion of the medical home concept, and to enhance the ability of state Title V agencies to promote, maintain and spread that improvement over time.
Who
The project involved nine Title V teams and 17 primary care practices.
Our Role
Led a Breakthrough Series learning collaborative to apply quality improvement methodology to implementing a medical home model of care. Provided an environment for rapid-cycle testing and the robust exchange of ideas with the support of world-class medical home and quality improvement experts.
Funder
The project was funded by the U.S. Health Resources and Services Administration, Maternal and Child Health Bureau and conducted in partnership with the National Center for Medical Home Initiatives for Children with Special Health Care Needs and the Center for Medical Home Improvement.

From the Alliance for Innovation on Maternal Health. PSBs are collections of evidence-informed best practices that address clinically specific conditions in pregnant and postpartum people. The NNPQC helps provide TA and support to PQCs in adopting the core AIM Perinatal Mental Health Bundle.
