Florida Learning and Action Network (LAN) and Asthma Care Team (ACT) Programs

Status: Complete

WHAT: Two projects working synergistically to improve collaboration between healthcare and public health agencies to progressively expand sustainable and comprehensive asthma control service networks across Florida. One project involves statewide, sector-specific LANs that include managed care organizations and providers. The second involves community-based, multi-sector ACTs that include providers, schools, hospitals, pharmacies, local public health agency and housing entities.

WHO: The LAN engaged managed care organizations and providers. The ACT engaged multi-disciplinary and multi-sector representatives including providers, schools, hospitals, pharmacies, local public health agency and housing entities.

WHEN: May 2015 to August 2015

FUNDER: The project was funded by the Florida Department of Health.

OUR ROLE: Provided quality improvement expertise in the planning and development of learning collaboratives to engage different stakeholders and community members in improvement work to enhance care coordination and asthma management.
